Replacement of Loss RC and PC

For All Registered Practitioner


Application for Replacement of Practising or Registration Certificate 

A registered person shall immediately apply to the Registrar of the Optometrists & Opticians Board (OOB), to duplicate a Registration Certificate (RC) or Practising Certificate (PC) under the following conditions:

    i.    A RC/PC has been lost

    ii.    A RC/PC has been destroyed

    iii.    A RC/PC has been defaced

    iv.    A RC/PC has been obliterated

    v.     Or when any particulars on the RC/PC is illegible

In the case of RC/PC which has been defaced or obliterated, the application shall be accompanied by the following supporting documents and prescribed fee:

    a. The original defaced or obliterated RC/PC.

    b. Application form for duplication of RC/PC.

    c. A non-refundable replacement fee of $50

In the case of RC/PC which has been lost or destroyed, application shall be accompanied by the following supporting documents and prescribed fee:

    a. An original copy of the duly signed and stamped statutory declaration* by Commissioner of Oath.

    b. Application form for duplication of RC/PC.

    c. A non-refundable replacement fee of $50

With effect from 1 February 2018, the Commissioner for Oaths (CFO) services at the Supreme Court will only be available for Supreme Court-related matters. Members of the public who require CFO services for non-Supreme Court related matter (such as to make a statutory declaration) can now approach private law firm for Commissioner for Oaths (CFO) services.

All CFOs charge a standard rate of $25 per statutory declaration.

Outcome of Application



1. We would only process application with full payment and a copy of the stamped statutory declaration. The processing time for each application is approximately 14 working days.

2. You will be notified via email for successful application. The Board will schedule an appointment for you to collect your new RC/PC in person which you are required to surrender the original statutory declaration form.

3. In the event that where any original RC/PC is found after any duplicate thereof is issued, the holder of the certificate shall immediately return the duplicate to the Board for cancellation.
