Allied Health Professionals (AHPs) who were previously granted conditional registration and had their names removed from the Register pursuant to Section 26 of the AHP Act may apply for restoration of their names to the Register of Allied Health Professionals with Conditional Registration if they wish to return to practice in Singapore.
Please be informed that the prevailing supervision requirements will apply for the application for restoration. This means that the prospective employer must be able to demonstrate its ability to provide the required supervision (i.e. completed the Readiness Review) under the Council’s supervisory framework and assigns the AHP on Conditional registration with a supervisor who meets the Council’s eligibility criteria for supervisors.
Submission of Application
The following documents must be submitted as part of your application for Restoration:
- Statutory Declaration; (to be duly signed and stamped by a Commissioner of Oaths or other officer empowered by law to administer Oaths)
- Letter of offer of employment from the prospective employer in Singapore;
- Letter stating grounds on which application is made
- Details of employment history; and
- Form SF2 - Undertaking by supervisor.
Satisfaction of the criteria for restoration does not mean automatic approval. Each application for restoration will be considered on its own merits.

We will contact you on the payment details after we have received the application form and supporting documents. Do note that payment will be via PayNow.
The processing time for each application will take a minimum of 2 weeks, upon complete submission of all required documents and information as determined by the Council.
